5-15-11: All COBRA & self payments should be received by our office NO later than the 22nd of the month PRIOR to the month of eligibility (eg. Payment for May eligibility should be received by April 22).

12-7-09: An updated SPD (Actives) is now available. The Deductible (individual and family) on the Self-Funded PPO plan (for in-network benefits) has been reduced to $0, and the out-of-pocket has increased to $1,250 for single and $2,500 for family effective 1/1/10.
